Request for Services (RFS) Digital Form Submission:

Please submit 4-6 weeks in advance of the event/trip
Please complete the form in one sitting.

If form submission was successful:
The submitter will receive a confirmation email
Your arts representative and superintendent will be notified of the request
Once approval has been granted, BOCES will begin processing the request
